Changes to registration during Covid-19 Pandemic
Due to the current COVID-19 pandemic, patients are asked to register without coming into the practice. To register you will need to complete our online registration form.

We welcome new patients to the surgery who reside within the catchment area. You can check if you are within our catchment area by entering your postcode below.
How to register
Please complete our online registration form, you may be asked to provide evidence of residency via email. Please bring your medical card to the surgery; if this has been mislaid we will still be able to help you.
As no one is automatically registered with a surgery, it is important that both you and all members of your household register at the same time. We recognise the important role of carers in looking after dependent relatives; please make it known if you are caring for someone.
Who will be your doctor?
Your registration will be with the practice - Lime Tree Surgery. You will be given a named doctor responsible for care however you can see any of the doctors practising at the surgery. Although you may have to wait longer to see a specific doctor, for continuity of care, we would encourage you to consult the same doctor as far as possible.
Your First Appointment
When you register with the practice you will be asked to complete a health questionnaire (one for each member of your household). This can be collected from our reception desk or downloaded below, printed and filled out to save you time. If you wish, you can make an appointment for a new patient check.
If you are ill while away from home or if you are not registered with a doctor but need to see one urgently you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered. We are unable to provide repeat medication to temporary registration patients, if you have any further queries please contact our reception team on 01903 264101.
