Information for our LGBTQ+ patients
At Lime Tree Surgery, we provide a service to meet the needs of patients over the age of 18 who identify as transgender, non-binary, gender queer, agender, intersex and gender fluid. We aim to improve access to services in the practice and improve your patient experience. Patients over the age of 18 who identify with as transgender, non-binary, gender queer, agender, intersex and gender fluid are invited to have an annual review of physical, mental, and sexual health, as well as support with hormonal therapy including prescribing and monitoring according to national guidance, refer to the Tavistock and Portman GIC guidance.
We are unable to support prescribing and monitoring of prescribing in under 18s, according to national guidance. We will, however, continue to support anyone under the age of 18 who may identify as transgender, non binary, gender queer, agender, intersex and gender fluid. We would refer to appropriate services on the NHS for under 18s as well as signpost to relevant supporting charities.
Changing Your Gender or Name in NHS Records – What You Need to Know
We support all our patients in expressing their gender identity and want to make sure you're aware of how changes to your NHS records are handled, including how these changes may affect screening invitations.
Patients Under 18
The NHS no longer allows changes to gender markers or NHS numbers for patients under 18.
You can still:
- Change your name
- Choose your title and preferred pronouns for use during appointments
These changes will be respected in how we communicate with you.
Patients Aged 18 and Over
You can request to:
Change your gender marker (e.g. from male to female) and/or receive a new NHS number
A Gender Recognition Certificate is not required.
If you choose to get a new NHS number:
- You’ll be registered as a new patient at the practice
- Some medical history may not automatically transfer
- You may lose your place on NHS waiting lists
- This change cannot be reversed

Changing Your Name
- You can change your name on NHS records.
- We will ask for documentation to support this part of our practice policy.
Screening Invitations and Gender Identity
Changing your gender marker may affect which NHS screening invitations you receive. Here’s how it works:
Cervical Screening
Only patients registered as female are automatically invited.
If you're registered as male but still need cervical screening (e.g. you have a cervix), you can opt in:
Speak to your GP or nurse
Or use the NHS Cervical Screening Opt-In Form
Breast Screening
Patients registered as female aged 50 to 71 are routinely invited.
If you’re registered as male but believe you’re at risk (e.g. due to hormone therapy or family history), speak to your GP for a referral.
Abdominal Aortic Aneurysm (AAA) Screening
Offered to patients registered as male aged 65+.
If you were assigned male at birth but are registered as female, you can still request this screening:
- Speak to your GP
- Or visit Find AAA Screening Services
Bowel Cancer Screening
Offered every 2 years to all patients aged 60–74 (gradually expanding to 50–74).
If you're over 75, you can request a test kit by calling 0800 707 6060.
