ADVANCED CLINICAL PRACTITIONER (SAME DAY ACUTE CARE)

We are seeking an experienced Advanced Clinical Practitioner (ACP) from a Nursing, Paramedic or Pharmacy background to join our Same Day Team. The successful candidate will provide autonomous assessment, diagnosis and treatment of acute and urgent presentations within General Practice, supporting same day access and improving patient outcomes through advanced clinical decision making and multidisciplinary working.

Job Title: Advanced Clinical Practitioner (Same Day Acute Care)

Profession: Advanced Practice Pharmacist, Advanced Nurse Practitioner or Advanced Paramedic Practitioner

Band: Equivalent to Agenda for Change Band 8a (dependent on qualifications and experience)

Hours: 36 hours per week, worked flexibly across 4 or 4.5 days.

Location: Lime Tree Surgery

Responsible To: Practitioner Lead

Salary: Equivalent to Agenda for Change Band 8a (£55,690 to £62,682 per annum pro rata), dependent upon experience, qualifications, advanced practice credentials and prescribing status. 

Overall Purpose

The Advanced Clinical Practitioner (ACP) will work autonomously within the Same Day Team, providing expert clinical assessment, diagnosis, treatment and care management for patients presenting with acute, urgent and undifferentiated conditions. The post holder will be an experienced Advanced Clinical Practitioner from a Nursing, Paramedic or Pharmacy background, educated to Master's Level in Advanced Clinical Practice (or working towards), with demonstrated competence in assessment, diagnosis and management of acute presentations within Primary Care. The ACP will support the delivery and ongoing development of a high-quality same day access service, helping to ensure patients receive the right care, at the right time, in the most appropriate setting whilst reducing avoidable hospital admissions and unnecessary GP appointments.

Key Responsibilities

1. Advanced Clinical Practice

The post-holder will:

  • Provide autonomous assessment, diagnosis, treatment, prescribing (where qualified) and management of patients presenting with acute, urgent, minor illness and undifferentiated conditions.
  • Deliver safe and effective same day care through face-to-face, telephone and digital consultations.
  • Undertake comprehensive clinical assessment, including history taking, physical examination and clinical decision making.
  • Formulate differential diagnoses and evidence-based management plans.
  • Request, perform and interpret relevant investigations and diagnostic tests.
  • Initiate appropriate treatment plans, including medicines optimisation and prescribing within scope of practice.
  • Safely refer patients into secondary care, community services or other healthcare pathways where clinically indicated.
  • Recognise deteriorating patients and manage medical emergencies appropriately.
  • Undertake home visits where required for acute assessment and management.
  • Manage clinical risk and uncertainty within an autonomous advanced practice role.
  • Ensure accurate, contemporaneous and legally compliant clinical records are maintained.
  • Provide health promotion, self-care advice and preventative interventions.
  • Act as a clinical resource for the wider multidisciplinary team.
  • Support achievement of same day access targets and effective demand management across the practice.

2. Clinical Leadership

The ACP will:

  • Demonstrate leadership across the Same Day Team and wider multidisciplinary workforce.
  • Provide clinical supervision, support and mentorship to colleagues where appropriate.
  • Act as a role model for advanced clinical practice and evidence-based care.
  • Contribute to service redesign, quality improvement initiatives and development of clinical pathways.
  • Support implementation of local and national clinical guidance.
  • Participate in significant event reviews, clinical governance and risk management processes.
  • Work collaboratively with GPs, Nurses, Pharmacists, Paramedics and external partners to improve patient outcomes.

3. Education and Professional Development

The post-holder will:

  • Maintain advanced clinical competence through ongoing CPD and professional revalidation requirements.

  • Research
  • Support the education and development of students, trainee ACPs and other healthcare professionals.
  • Participate in teaching, mentoring and supervision activities.
  • Contribute to the development of local guidelines, pathways and protocols.

4. Communication

The post-holder will:

  • Communicate effectively with patients, carers and multidisciplinary colleagues.
  • Explain complex clinical information clearly to support shared decision making.
  • Produce high-quality clinical records, referrals, reports and documentation.
  • Participate actively in practice and neighbourhood team meetings.
  • Build effective relationships with primary, community and secondary care services.

Person Specification

Essential

  • Registered Healthcare Professional: 
  • NMC Registered Nurse; or
  • HCPC Registered Paramedic; or
  • GPhC Registered Pharmacist.
  • Master's Degree in Advanced Clinical Practice (or working towards with substantial progress demonstrated).
  • Evidence of advanced clinical assessment and diagnostic skills.
  • Independent Prescriber qualification (or willingness to obtain within agreed timeframe).
  • Experience managing acute and undifferentiated presentations autonomously.
  • Experience working within Primary Care, Urgent Care, Emergency Care or Community Services.
  • Excellent clinical decision-making and risk-management skills.
  • Demonstrable commitment to continuous professional development.

Desirable

  • Experience working within General Practice.
  • Non-Medical Prescribing qualification already in place.
  • Minor illness and/or minor injury qualification.
  • Experience supervising or mentoring staff.
  • Experience leading service improvement projects.
  • Completion of a recognised Advanced Clinical Practice programme aligned to NHS England ACP Framework.

A Patient Participation Group (PPG) is formed to enable patients to engage with the practice to develop the surgery’s activities to improve patient experience and contribute to Lime Tree’s mission of supporting all of our patients with their health and wellbeing.

It is of great importance to us that our developing PPG is representative of our diverse population and that those who choose to apply to be part of this valuable group embody the values of the practice: respectful, approachable, professional, dignity, holistic, compassionate and supportive.   

We are hoping to recruit a broad range of patients from our whole catchment area and would particularly encourage you to express interest if Durrington Health Centre is your closest surgery site.         

To ensure that the group is representative of our patient demographics we are looking to recruit Patient Champions who fit one or more of the following definitions:

  • Young person aged 18-30
  • New parent (those with a child aged under 2 years)
  • Carer (unpaid)
  • Care leaver (i.e. someone who has lived within care)
  • Neurodivergent or learning disability
  • Lived experience of a mental health condition
  • BAME
  • LGBTQ+

What to expect

We expect the PPG to evolve based on its membership and with the support and guidance of the practice management. The practice will consult the members for feedback on current service provision as well as asking for ideas and feedback on developments that the practice are considering.

In time we would also like the PPG members to be able to use their own interests, skills and experiences to provide activities and support to their fellow patients such as: support with digital access, walking group, chatty café and befriending, reading group…

It is important to note that you shouldn’t join a PPG as a vehicle for complaining about service provision or to advance your own receipt of care. Rather, being a member of the PPG involves being a selfless and critical friend to the practice.

Level of commitment required

Being part of the PPG shouldn’t be seen as an onerous task but a certain level of commitment is required. The group will need to meet with the practice 6 times a year through a mixture of face-to-face and virtual meetings as well as some email or similar communication between meetings.
