Support available at Lime Tree Surgery
Lime Tree Surgery offers support to patients who would like help with weight management and improving their health.
We recognise that weight can be affected by many factors, including health conditions, medication, lifestyle and personal circumstances. Our approach is supportive and personalised.
If you would like support with your weight, please contact the practice.
Weight Management Review
A review may include:
• Height, weight and BMI measurement
• Lifestyle review including diet and physical activity
• Discussion of weight-related health risks
• Support with setting achievable goals
• Healthy lifestyle advice
• Signposting or referral to appropriate services
BMI is considered alongside your overall health and individual circumstances.
Annual Weight Management Review
Patients living with obesity may be offered an annual review.
This may include:
• Weight and BMI review
• Review of progress and lifestyle changes
• Review of weight-related health conditions
• Discussion of ongoing support options

West Sussex Wellbeing provides support with:
• Weight management
• Healthy eating

Gro Health Digital Weight Management Programme
A digital weight management programme for eligible West Sussex residents.
Support includes:
• Health coaching
• Nutrition support
• Exercise guidance
• Lifestyle support

NHS Digital Weight Management Programme
Some patients may be eligible for NHS digital weight management support.
Eligibility may include:
• Age 18 years or over
• BMI 30 or above
(BMI 27.5 or above for people from Black, Asian and minority ethnic backgrounds)
• A diagnosis of diabetes (type 1 or type 2), hypertension, or both
Some services may not be suitable for people who:
• Have moderate or severe frailty
• Are pregnant
• Have an active eating disorder
• Have had bariatric surgery within the last 2 years
• Have circumstances where the risks may outweigh the benefits
Eligibility will be assessed by the clinical team and depends on NHS criteria and service availability.
Specialist Weight Management Support
Some patients may require additional support and may be considered for referral to specialist services.
This may include:
• Specialist dietetic support
• Psychological support
• Medical weight management advice
Your GP or practice team will advise if this is appropriate.
